PLAN OF CARE
An individualized plan of care is developed for each patient, identifying his/her special medical and physical needs and strengths. The interdisciplinary team works with the patient to determine rehabilitation goals and approaches. We aggressively monitor the progress of each resident to ensure they are the path toward these goals and approaches. Our objective is to help patients achieve their highest possible level of function and independence.
INTENSIVE REHABILITATION
Rehabilitation is an important part of the recovery process. The combined efforts of our physical, occupational and speech therapists with rehabilitation nursing, social services and dietary teams provide the necessary components and support for the successful rehabilitation. In addition, we employ our own physical therapists, occupational therapists and speech pathologists. That means patients receive a plan of care that will be given by the same team, creating a familiar trust level for the patient.
Physical therapy is designed to meet the needs of those patients who have lost functional independence as a result of injury or illness. This therapy helps patients regain balance, mobility and walking skills and recover from painful injuries.
Occupational therapy provides therapeutic activities intended to help patients achieve the highest level of independence in daily life. It teaches self care skills, mobility, toilet training and bathing and aids in easing emotional and social adjustment.
Speech therapy is geared specifically for people with speech, language, swallowing and hearing disorders. The Speech Pathologist works with safe swallowing and feeding techniques, memory enhancement training, social skills training and expressive language training.
SPECIALIZED NURSING CARE
The transition from hospital to home is further enhanced by a nursing staff trained and experienced in both medical management and rehabilitation. Following physician’s protocols, our professional nursing team provides medical assessment and monitoring, medication and pain management as well as rehabilitative support. Our full time wound-vac specialists make our center an extremely successful location for patients with wound healing requirements. Assisted Living is the ideal solution for those who need some support in daily living.
At Emerald Cozad we focus on residents getting the help they need to maximize their independence by providing assistance as well as daily activities. We strive to provide a home-like environment in many different ways, such as pet visits and recreational outings. Our activity staff continues to create exciting programs that fit specifically to each elder’s interests.
Assisted Living elders are independent in their mobility and are able to provide for their own activities of daily living. They may need minimal attendant care or supervision.
